An employee was placing a board on top of a bundle of steel tubing. A crane was moving a bundle of steel that struck the employee and caught them against another bundle of steel tubing. The employee sustained a crushing injury and hematoma to the right upper leg.

An employee was helping a crane operator rig a chain around two bundles of steel tubing. The hoist was raised, and the employee's thumb was caught between the chain and a steel tube. The employee suffered a broken thumb and damage to the thumbnail.

An employee was cutting 6-inch round stock (steel) in a horizontal scissor-style band saw. A 16-inch long cut piece was being manually repositioned for lifting when it shifted. The piece caught the employee's left middle and ring fingertips against the frame of the band saw. The employee's middle fingertip was amputated. They also sustained a crushing injury to their ring finger and loss of a fingernail.

An employee was using an overhead crane to align a tooling rack. The employee's hand slipped and the tooling rack shifted, pinching his right fifth finger. The employee's fingertip was amputated.

An employee was handling scrap metal at a machine when they stepped on a piece of scrap metal and it punctured their right foot. The employee sustained a laceration to the foot.

An employee was fixing a mechanical casket lift in a vehicle when the lift came down on their left-hand fingers, resulting in amputation of two fingers.

The employee's right foot was crushed and all of his right toes were amputated.
