An employee was helping a crane operator rig a chain around two bundles of steel tubing. The hoist was raised, and the employee's thumb was caught between the chain and a steel tube. The employee suffered a broken thumb and damage to the thumbnail.

An employee was cutting 6-inch round stock (steel) in a horizontal scissor-style band saw. A 16-inch long cut piece was being manually repositioned for lifting when it shifted. The piece caught the employee's left middle and ring fingertips against the frame of the band saw. The employee's middle fingertip was amputated. They also sustained a crushing injury to their ring finger and loss of a fingernail.

An employee was using an overhead crane to align a tooling rack. The employee's hand slipped and the tooling rack shifted, pinching his right fifth finger. The employee's fingertip was amputated.

An employee was handling scrap metal at a machine when they stepped on a piece of scrap metal and it punctured their right foot. The employee sustained a laceration to the foot.

An employee was preparing two steel strip ends to weld together. While he was shearing the end of a coil off, his right middle fingertip was amputated, and his right ring fingertip was severely lacerated, by a side clamp plate and a steel strip edge.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.

An employee was stacking concrete blocks for a concrete bunker. As a block was swinging into place, the employee's left leg was pinched between two blocks, resulting in injuries to his left heel and lower leg.

An employee was using a crane to move a 44-foot, 3,343-pound I-beam. As the beam was moving west, it straightened out (north to south), beginning to swing south toward the northwest corner of a building. The crane then sent the beam southward, directly toward the corner of the building. The beam caught the tips of the employee's left index, middle, and ring fingers against the building. The last joints of the middle and ring fingers were amputated, and the last joint of the index finger was partially crushed.

Employees were using a crane to lower large tree stems and branches while removing limbs and branches from the load. This caused the load to shift and strike an employee in the back, resulting in a hematoma on the lower left part of their back and a fractured ankle.

A metal tooling base was being moved by a forklift when the load shifted. An employee who was assisting went to stabilize the load and prevent it from sliding off the forks. The C-channel of the base crushed the employee's left hand against the concrete ground, resulting in the amputation of their third and fourth fingers.

An employee fell onto a hot billet and suffered a burn to the left leg. The employee was hospitalized.

An employee was cutting wooden boards using a table saw when the glove on his left hand was caught by the blade, resulting in a partial fingertip amputation.

A crane was hoisting a pipe. An employee was adjusting twisted lifting chains on the end of the pipe when the pipe was raised, causing the employee's hands to be caught between the chains and the pipe. The employee suffered injuries to the left index finger, left ring finger, right ring finger, and right index finger; the right index fingertip was amputated without bone loss.

During a routine inspection, an employee placed their hand on a pipe to check a weld. The pipe clamp activated, closed on the pipe, and trapped the employee's right middle finger, resulting in a partial amputation and fracture to the fingertip as well as a laceration to the ring finger.

An employee was working to gain the attention of a coworker. He reached over a hot steel pipe and his shirt got stuck on the coating. This caused a loss of balance and he contacted the hot pipe. The employee sustained second-degree burns to his right elbow, left hand, and abdominal area.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.

An employee was shot in the leg with a pistol during an altercation with two people who had entered the store without permission.

An employee was directing a truck driver to a dump site when the truck's peanut trailer slipped off the kingpin. As the trailer fell, a ladder attached to it lacerated the employee's ear. The employee was hospitalized.

An employee was unloading a forklift from a trailer to the ground. The employee was climbing down the ladder of the trailer when their foot slipped through one of the rungs, causing them to fall backward onto the forklift forks. The employee was hospitalized for fractured ribs.
