An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was descending a stairway when they lost their balance. The employee went to catch themselves and their finger became stuck between two angles and degloved. The employee sustained a medical amputation of their ring finger to the second knuckle.

An employee was descending a set of inclined ship stairs when she slipped and fell, breaking her right ankle.

An employee was working on an amphibious assault ship. While exiting the work area, she was climbing over a chill water pump when her leg/foot got caught on the pump. She stumbled and struck her right lower leg/ankle against a steel cutout. The tibia and fibula were fractured where they connect to the right ankle.

An employee was fitting a T-beam into place with an overhead crane. The T-beam was moved into position under another one. The employee then attempted to lower the hoist and slack the line so the T-beam could rest freely. The crane raised the hoist instead, causing it to snag under the second T-beam, then break loose and strike the employee in the chest and face. The employee suffered fractured ribs, a punctured lung, and a fractured jaw.

The injured employee was holding a board that was being cut by another employee. The board kicked back, causing the saw to amputate the employee's left little finger and lacerate their left hand, resulting in nerve damage.

On September 26, 2023, an employee was working in a trench box while a co-worker was lowering a piece of plastic pipe by hand using a rope. The pipe swung and struck the employee's hand, fracturing their right thumb and middle and ring fingers. The employee was hospitalized.

An employee went over to a co-worker's station while the co-worker was using a powered meat knife. As the injured employee was pointing, his right index fingertip was amputated.

Two employees were using a handheld banding tool to make banding tape for tent set-ups. One employee was holding the banding tool and the injured employee was looping a metal clip around the tape. The tool closed while the injured employee's hand was near the banding mechanism. The banding tape caught the employee's right ring finger as it tightened, resulting in a partial amputation.

An employee was holding a branch against a log on the ground to make a wedge while another employee was using a chainsaw to cut the wedge. The saw slipped and struck the employee holding the branch, resulting in a laceration to the right wrist.

An employee was descending a ladder carrying a bag of trash. He fell, landed on the ground about 10 feet below, and suffered fractures to his right hip and pelvis.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.
