On 06/24/16, at 5:00 p.m., an employee had been working on paper machines when he began feeling sick. He sat in the break room to cool down and drink water. He later felt nauseous and began vomiting, requiring hospitalization for dehydration.

An employee was resetting the chain at the core station. He stepped onto the foot pedal that raises and lowers the hoist as he grabbed the chain and sustained amputation of his left index and ring fingers.
