An employee was working on a jet wheel blaster and putting dies into the machine when their thumb was caught between the moving table and a fixed part. The employee sustained a crush injury to the finger.

An employee was maneuvering steel bundles supported by an overhead crane. The employee's right hand was caught under or beside the bundle, and a fingertip was amputated at the last knuckle.

An employee was doing electrical work. An arc flash caused the employee to suffer third-degree burns to the chest and hands.

An employee had three baskets of parts stacked on a work table. The top basket slid off the table and lacerated her right thigh.

An employee was organizing merchandise in the backroom. She lifted a box of merchandise and when she went to put the box down, she sustained back pain.

An employee was unloading boxes from a shipping container by hand when he felt a pinch in his back. The employee was hospitalized for a subarticular disc protrusion.

An employee was lifting boxes and pails when their back began to hurt. They were hospitalized for two slipped discs and inflammation.

An employee was setting up a meal service for a new hire orientation. The employee was removing food items from a speed rack station and bending to retrieve bowls of food when she sustained a back strain.

An employee was transferring a patient from a chair to a bed and strained their back.

An employee was using a hydraulic bearing puller (held by a chain hoist) to remove a bearing from a railroad axle. The hydraulic bearing puller fell from the hoist (approximately 18 ) and struck the employees left foot, fracturing it.

An employee had been making deliveries. While unloading material from a tractor trailer, the employee sustained symptoms of heat-related illness. The employee was hospitalized.

An employee was loading/unloading product from a trailer at a customer location when he fell 10-12 feet from the top of the trailer to the concrete ground below. The employee sustained a fractured left ankle.

The employee was climbing into the driver side of the truck in the parking lot while holding a lunchbox and a clipboard. The employee slipped on snow and fell to the ground, fracturing a hip.

An employee was making a delivery and was pulling the customer's freight to the rear of the trailer using the plastic strap around the freight. The strap broke, causing the employee to fall from the rear of the trailer onto the asphalt pavement below. The employee sustained fractures to their left shoulder, elbow and pelvis.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
