A packaging machine vacuum stopped working. The employee thought the vacuum line was clogged and reached inside the machine with his right hand to unclog it. The fan blade was still rotating and struck his right middle finger resulting in a fingertip amputation.
An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool.
