An employee was walking around a customer's establishment looking for a floor mat that was to be cleaned when the employee tripped over a duffel bag and fell to the floor, suffering a fractured hip.
An employee was working on an industrial dryer when the dryer door fell due to a defective air blocking valve that normally holds the door in place. The employee was struck in the neck and shoulder by the dryer door and sustained fractures to his C1 and C3 vertebrae, a spinal compression at the C2 vertebra, and a bruised right shoulder.
