An employee was inflating a tubeless lawn mower tire utilizing a lock-on air chuck to seal off leaks and seat the bead. To verify the bead was successfully seated, the employee placed water on the seam of each side of the tire. He noticed a small leak in the seam on the side opposite the valve stem and the employee added more air to seal off the leak. The tire was laying flat on the ground with the valve stem facing up. As he removed the air chuck from the tire, the seam failed and blew the tire and rim upward, striking the employee in the forehead. He was hospitalized for a forehead laceration.

An employee was performing finishing work on a residential driveway when they began to experience body cramps and were hospitalized for heat stress and dehydration.

An employee was staking tomato plants on a farm. The employee began to feel sick and collapsed, suffering from dehydration and heat shock.

A second operator had slightly clamped around the pipe to keep it in place. The third operator called out a measurement. The machine actuated and caught the the injured employee's left middle and ring fingers between the wiper and the material. The employee required surgery and sustained amputations to the distal joint of the middle and ring fingers.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

A security employee was holstering her firearm when the gun discharged a bullet into her right leg. The employee was hospitalized.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.
