An employee was greasing a conveyor when his foot slipped from the framework he was standing on and ended up on the chain drive and sprocket. The drive and sprocket crushed the toe box of his boot, partially amputating his right big toe.
An employee was operating a trimmer from a control panel when a chain came off the sprocket. After replacing the chain on the sprocket, he stepped out of the system, lost his footing, and fell 14 feet to the concrete floor. The employee sustained a broken pelvis and back.
