A flagger was walking behind a mini-excavator. The excavator backed into him and fractured his heels, his left ankle, and his left wrist, as well as injuring a tendon in his left thumb.

BRUNDAGE-BONE CONCRETE PUMPING, INC.

At 4:00 p.m. on June 13, 2025, a concrete truck was parked behind a concrete pump, discharging its material; meanwhile, a Brundage-Bone Concrete Pumping employee was standing near the rear of the pump, checking the machine and the material to be placed. The truck's transmission engaged, and it reversed and pinned the employee against the pump.
