A flagger was walking behind a mini-excavator. The excavator backed into him and fractured his heels, his left ankle, and his left wrist, as well as injuring a tendon in his left thumb.

The truck's transmission engaged, and it reversed and pinned the employee against the pump. The employee suffered broken ribs, a severe laceration to the right hip and leg, muscle damage, and lacerations to the face and head. He was hospitalized, requiring surgery for the hip laceration.

An employee was raking and shoveling dirt next to a motor grader when it reversed and the wheel turned, causing the tire to strike the employee. The employee was knocked down and the grader ran over his leg and foot, fracturing them.

An employee was struck by a skid steer that was backing up, knocking them over. The track of the skid steer went over his lower half, resulting in a fractured pelvis.

An employee was working as a spotter for a tree truck that was being parked near a tree. The employee was behind the truck when he was pinned between the truck and the tree. The employee sustained a lacerated liver.

An employee was using a hook tool to pull a pallet onto the lift gate of a tractor trailer. The hook detached from the underside of the pallet, causing the employee to fall backward off the lift gate. The employee landed on the concrete parking lot about 5 feet below, suffering fractures to the skull and two thoracic vertebrae.
