Final narrative
On July 19, 2016, at approximately 7:10 p.m., an employee was loading a tube in the tube bender when a clamp closed on the employee's right fourth and fifth fingers. A portion of the fourth finger was surgically removed the following day.
An employee went to a truck to get a drill to fix a door. After retrieving the drill from the truck, the employee turned to close the truck door. His index fingertip was then caught and amputated between the truck door and the truck door frame.
