Final narrative
On 7/19/2016, an employee was performing maintenance on a palletizer lift elevator. The lift platform was elevated about 3 inches with a chain fall in order to adjust the lower jack bolts. The elevator dropped, crushing the employee's left index finger between the jack bolt and palletizer elevator resulting in an amputation close to the second knuckle on the left index finger.
