On July 20, 2016, at approximately 12:30 pm, an employee was in the process of moving a loaded barge with a cable that travels through a sheave. The employee's hand became caught in the sheave amputating the employee's right index, middle, and ring fingers at the first knuckle.

An employee was conducting preventive maintenance on a decline conveyor, spraying lubricant on its tension rollers, when his left arm was caught by the conveyor system and pulled between the roller and the belt. He suffered a fracture to the wrist and two fractures to the forearm.

An employee was bringing a filter online. When the employee opened a valve to initiate flow through the filter, the gasket on the filter housing failed, spraying a water/erythritol mixture (heated to about 175 degrees Fahrenheit) toward the employee. The employee moved back quickly and their chemical suit caught on something, causing it to open. The mixture burned the employee's abdomen and left arm.

An employee was replacing the exhaust fan above the scalding area when their finger became pinched in the exhaust fan shroud. The employee sustained amputation of the distal tuft of their right middle finger with bone loss.

On July 17, 2024, an employee was trying to retrieve his knife from the bottom of a sterilizer. Water in the sterilizer, heated to 180 degrees, entered the employee's right glove and burned his palm and the top of his hand. He was hospitalized.

An employee was waiting for the elevator to get to the floor that he worked on. The employee went to step through the elevator door, but the elevator lift was not there, causing him to fall 20 feet to the basement below. The employee sustained fractures in both legs, the right heel, and lower vertebrae. He was hospitalized.

The employee's wrench slipped off the equipment, causing them to lose their balance and fall. The employee landed on the ground 14 feet below and suffered fractures to the orbital bone, left elbow, and lower back.

An employee was walking past a crossover conveyor when a ramp came down and hit them in the back. The employee sustained a broken back vertebra, as well as a concussion, and was hospitalized.

An employee bent over to pick up a rubber mat from the ground. When he grabbed the corner of the mat, a rattlesnake hidden under it bit his left middle finger near the first knuckle. He was hospitalized.

An employee had been cleaning a grain bin. He walked across the bin and his left foot contacted an unguarded floor auger, resulting in severe lacerations to the foot. The auger was not locked out/tagged out at the time.

On September 23, 2025, an employee was monitoring a feed roller when they tripped and their right hand contacted the roller. The employee was hospitalized and their right hand was amputated at the wrist.

U.S. Postal Service - Northfield Post Office

An employee was walking in a garage toward her delivery vehicle. As she stepped down a curb near her vehicle, she fell and landed on the concrete garage floor, resulting in a hip injury that required hospitalization.

Loram Maintenance of Way, Inc.

Employees were performing maintenance operations on a railroad line when the machine experienced a load conveyor motor failure. While clearing the machine to allow for repairs, an employee pinned the left side of the swing conveyor. The operator on the right side then requested a slight bump to the left to align the right side for pinning. Once the swing was in position, the operator went to pin the right side and the chain tensioner failed, resulting in an abrupt release of the swing chain. The chain snapped back and struck the operator s right hand, resulting in a 1-inch laceration and a fractured fifth metacarpal. The employee was hospitalized and required surgery on the hand.

An employee was decommissioning a substation. The tower switch was being set down on the ground when the switch blade moved. The employee's finger was crushed and amputated.
