Final narrative
On 7/22/16, at 10:30 a.m., an employee was setting up a shear to shear parts and was adjusting the stops. During the setup, the shear cycled and his left middle finger was caught in the pinch point between the bench and the part hold-down, amputating his fingertip.
Employees were assembling and testing a butterfly valve unit and actuator combo. When the unit was function tested with air, the valve gate closed on an employee's left thumb resulting in a fracture and laceration.
