Final narrative
An employee was on a 3-foot ladder guiding cable onto a spool during installation on an overhead door. As the control box was jogged, the employee's hand was caught between the cable and spool, resulting in a wrist and hand fracture.
An employee was operating a hoist to move steel I-beams and set up an area. As he was moving an I-beam, it bumped into another I-beam and caused it to fall over. The falling I-beam landed on the employee's left leg, breaking it. He was hospitalized and had surgery.
An employee was using an overhead crane to move a 3/8" thick, 3' x 10' piece of steel plate. There was a chain on the crane hook, which was attached to an eye bolt for moving the steel plate.
