The worker was performing a demonstration event. She walked over to the produce section to get grapes for her demonstration. She was in front of the grapes, reached for an item, turned to go back to her demo cart and fell onto her right side, fracturing her right hip.

An employee was on a forklift that was elevated approximately 20 feet and was auditing inventory. The forklift malfunctioned and the employee fell down to the surface below, causing them to sustain multiple fractures.

An employee was moving cereal boxes on shelves while on a step stool. The employee mis-stepped while descending the step stool and fell to the floor. The employee sustained a fractured left hip and injury to the left wrist and left leg.

The employee climbed to the back of the sign on the affixed metal lattice. The bolt came loose and the iron gave way, causing the employee to fall approximately 13 feet to the ground. The employee sustained fractures to their hips, arm, and shoulders. They also sustained torn rotator cuffs in both shoulders. The employee was hospitalized and required surgery.

An employee was climbing a hook ladder. A piece of advertising material fell onto the employee, causing them to slip and fall from the fourth or fifth rung. The employee landed on their left arm, breaking the arm and also suffering a broken rib.

An employee set up and secured an extension ladder on a pole so he could tighten up a low-hanging triplex cable. While on the pole, the employee started to pull the triplex cable to raise it up. The tension caused the pole to snap. The employee jumped off the ladder to avoid getting hit by the pole. The employee impacted the ground and sustained bilateral talus fractures.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
