An employee was in the process of removing an overhead roll-up door. While the employee was removing bolts from the door, the "D-Ring" swung out of place and struck the employee's ladder, causing the employee to fall and break an arm.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
