An employee was unloading shower doors from a straight truck when several pieces of glass in the truck fell over and pinned the employee against a rack with boxes on the other side of the truck. The employee suffered broken ribs, a bruised sternum, and a punctured lung.

The employee was pushing a cart with glass in it. The employee turned the cart and was about to hit a fan that was mounted to a pole. Other employees yelled for him to look out. The yelling startled the employee and he bent forward cutting his right bicep in the process. He was taken to the hospital for treatment.

An employee was pulling on a bale of corrugated cardboard boxes that were on a conveyor. The straps on the bale failed and the employee fell to the floor. The employee was hospitalized and required surgery for a fractured femur and/or hip.

As they entered, the self-closing overhead door shut while the injured employee's right hand was still on the edge. The employee's little fingertip was caught and partially amputated between the door and the frame.

An employee fell from the top of a fixed ladder, passing through the ladder's access opening and landing on the floor about 6-7 feet below. He suffered nine broken ribs (four on the left side and five on the right), as well as a punctured lung. He was hospitalized.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.
