An employee was talking to a coworker on the west side of the drive aisle. After finishing the conversation, the employee crossed back over to the east side of the drive aisle and was struck by a fork truck driving forward with a single material container. The employee was knocked to the ground and was struck a second time when the material container slid off the forks. The employee suffered a right thumb fracture, right tibia fracture, and lower leg scratch.

Rolling Frito-Lay Sales, LP

On December 30, 2023, an employee was pulling a pallet into the back room when a forklift rounded the corner and ran over the employee's right foot. Three of the employee's toes were amputated.

An employee was unwrapping a pallet of empty buckets. The employee stepped backward and was struck by a powered industrial truck carrying a pallet of bucket lids, resulting in a left ankle fracture.

An employee was climbing a ladder to get on top of the breakroom. The ladder slipped, causing him to fall to the ground. The employee sustained fractures to his upper back, left foot, and ankle.
