An employee was cutting a vertical pipe to remove a housing to a filter. After cutting the housing loose from the pipe, the employee removed the chain fall that had been used to raise the pipe. He attempted to shift the pipe slightly to replace the housing. The pipe fell straight down, striking and fracturing his left middle and ring fingers. He required surgery.
An employee was moving a piece of an I-beam for welding when it rolled off the cribbing. The employee went to catch it when it fell and crushed the employee's finger, resulting in a partial amputation.
