On August 25, 2025, a machine operator was working on the heat-set end of a textile bleach/prep range machine. He was running leader cloth and it was bunching up. He climbed a catwalk so he could reach the cloth and flatten it out. His right hand was caught in a nip roll while adjusting the cloth. The employee's middle and ring fingertips were amputated.

An employee was loading a ball warp onto a cart. The cart tilted and the ball warp fell on him causing a fractured left pelvic bone.

While stepping down from a step, an employee lost his footing and fell to the ground. His arm was broken, and he was hospitalized.

An employee was working on a slasher when the beam on the slasher moved and lacerated his forearm. The beam was not secured at the time.

An employee was attempting to repair a machine and got his hand(s) caught in the machine. The right pinky finger was surgically amputated.

An employee was patrolling a store using an electric patrol scooter when they lost their balance and sustained an ankle fracture.

An employee was operating a forklift to move panels around the dock area when the vehicle struck a joint where the two floors meet. The employee lurched forward and struck his head on the steering wheel. The employee then fell from the vehicle and sustained a dislocated disc in their back, requiring hospitalization.

An employee was standing inside a semi-trailer when the driver of the semi-trailer pulled away, causing the employee to fall backward and strike their head on a steel dock plate. The employee suffered a skull fracture, a fractured L1 vertebra, a broken coccyx, a laceration/abrasion to the head, and a brain bleed.

An employee was riding an ATV, catching up with a stray cow to return it to a pen. The ATV tire struck a hole or other obstacle, causing the employee to be ejected and fall to the ground. His leg was broken and he was hospitalized.

An employee was driving a van in reverse when they fell out of the vehicle and landed on the ground in the parking lot. The employee sustained a fractured left hip.

On September 20, 2025, an employee was clearing a jam on a blow mold machine. The employee's right index and middle fingers were crushed in the mold as it closed during a cycle. The middle finger was medically amputated.

An employee was lifting the chain latch on a gate that was stuck. The latch contacted their right middle finger, resulting in an amputation.

An employee was operating a pallet jack to transport heavy palletized loads through the department when the pallet jack slid on the wet floor. The injured employee was walking nearby and the pallet jack crushed their foot, resulting in a fractured foot bone.

An employee was scanning a member's purchase when a box containing septic treatment powder was dropped, causing a cloud of the powder to eject. The employee inhaled the chemical and was hospitalized with throat irritation and difficulty breathing.

An employee was stepping out of the building to go to her car when she slipped on ice and fell. The employee suffered an ankle fracture.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
