An employee was kneeling on the ground while fastening metal studs together using a cordless impact gun. While driving a self-drilling framing screw into a metal stud, the drill point portion of the screw snapped off. This caused the impact gun to slip forward and push the remaining portion of the screw through the employee's glove and into his palm.

An employee was cutting wood with a miter saw when the wood kicked out, causing his left hand to contact the saw blade. His hand was lacerated and fractured, requiring hospitalization and surgery. The saw was guarded and proper PPE was worn at the time.

An employee was power washing a metal tank from an elevated platform. They fell through a gap between the platform and the tank. The employee landed on the ground and sustained fractures to their ankles and feet.

An employee was installing a metal roof when he stepped on a skylight. The employee fell through the skylight to the ground, resulting in head and shoulder fractures.

An employee was replacing an HVAC unit in a residential home. While he was in the attic space above the garage, the wood he was walking on broke, causing the employee to fall through the sheetrock to the concrete floor below. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.
