An employee operating a stand-up forklift was struck by (broad side) another stand-up forklift at an aisle intersection. The collision pushed the forklift into a concrete barrier. The employee operating this forklift had his foot and ankle crushed between the forklift frame and the concrete barrier. The employee was transported to the hospital and hospitalized for surgery to the ankle and foot. The second forklift operator was not hurt.

Apex Industrial Services

An employee was traveling down an aisleway in a three-wheeled golf cart. They were moving around a parked forklift when a material cart came from the opposite direction. A wheel on the material cart caught a piece of racking causing the material cart to swing out and hit the injured employee's golf cart. The material cart continued to swing, hitting the injured employee's left leg. The injured employee was also knocked out of the golf cart and landed on the ground 2-3 feet away. The injured employee sustained fractures to the left leg, requiring hospitalization and surgery.

An employee was riding on a flatbed trailer when his left foot was caught between the trailer and a trackhoe. He sustained a laceration to his left foot that required stitches and hospitalization.

Employees were operating a baggage tractor on a service road. The tractor made contact with an aircraft's left wing and the seat of the tractor was ripped off. The employees were thrown to the ground. Two employees were hospitalized with injuries including a chipped tooth, bruises and abrasions, a dislocated hip, a fractured humerus bone, and a laceration to the left knee.

An employee was driving his vehicle (golf cart style) to an aircraft when he passed by two large baggage carts and was T-boned by another vehicle. He was hospitalized with a broken left leg.

An employee was operating a forklift, which had a T-bone collision with another forklift. The employee suffered four broken right ribs, a bruised right lung, and skin bruising and was hospitalized.

An employee was assisting with maintenance and servicing a conveyor that was making an unusual noise. The employee's right arm got caught in the conveyor between a belt and a roller. Their right hand and arm were crushed, resulting in amputation of the arm.

An employee was using a skid loader to clean an area of loose debris (straw shed). He was lowering the bucket when the arms/bucket of the skid loader pinned his feet against the step of the loader. The employee sustained fractures to both feet and was hospitalized.

An employee was stepping over a brace bar when they tripped and fell to the concrete floor, resulting in a fractured forearm and dislocated elbow.

An employee was on a step ladder loading product. They fell off the step ladder and sustained a fractured back and hip.

A Merit Logistics employee was pushing an egg crate out of a trailer. While working to get it over the lip of the dock, she lost her footing. Crates fell on her, breaking her tibia.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

General Thermodynamics, Inc.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

Sargento Foods Inc

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

Asher Enterprises, Inc.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
