On November 11, 2020, an employee was working from an extension ladder when it slipped. The employee fell about 6 feet onto a pallet of bricks below, suffering bruises to the back.
On November 10, 2020, an employee was on the roof of a two-story, new-build home to install anchor points for fall protection devices. The employee slipped on the wet underlayment and fell 20-24 feet to the ground below, suffering a fractured neck, ribs, and arms.
