Final narrative
An employee was putting rivets on the nose of a trailer while working from a platform. The work platform is approximately 36 inches wide, 6 feet long, 36 inches off the ground, with two steps to access. He was stepping down from the platform when he lost his balance and fell backwards, breaking his legs between the ankle and knee.
On June 21, 2016, a maintenance employee was working from a 6-foot ladder to change a light bulb when he received an electric shock. He fell to floor and broke his right hip, requiring hospitalization.
