An employee was hoisting an axle in the buggy department when one side of the sling came loose, causing the axle to tip. The sling hook raised up from the weight transfer, causing the employee's left little finger to be caught by the rigidness of the hook. The fingertip was amputated.

An employee removed a semi-refrigeration unit from the semi-trailer to repair it. After repair, the employee was putting it back on the semi-trailer, but the bolts did not line up. As the employee tried to adjust the bolts, his left index finger was caught between the refrigeration unit and the semi-trailer resulting in amputation of his nail and a quarter inch of his finger.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.

An employee was stacking concrete blocks for a concrete bunker. As a block was swinging into place, the employee's left leg was pinched between two blocks, resulting in injuries to his left heel and lower leg.

An employee was using a crane to move a 44-foot, 3,343-pound I-beam. As the beam was moving west, it straightened out (north to south), beginning to swing south toward the northwest corner of a building. The crane then sent the beam southward, directly toward the corner of the building. The beam caught the tips of the employee's left index, middle, and ring fingers against the building. The last joints of the middle and ring fingers were amputated, and the last joint of the index finger was partially crushed.

An employee was placing a board on top of a bundle of steel tubing. A crane was moving a bundle of steel that struck the employee and caught them against another bundle of steel tubing. The employee sustained a crushing injury and hematoma to the right upper leg.

Employees were using a crane to lower large tree stems and branches while removing limbs and branches from the load. This caused the load to shift and strike an employee in the back, resulting in a hematoma on the lower left part of their back and a fractured ankle.

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.

An employee was assisting with rigging a magnet to an overhead crane in order to lift a sheet of steel. The employee's right thumb was between the crane's hook and the chain passing through the hook, and when the crane was raised, the distal end of their thumb was crushed. Part of the employee's right thumb was medically amputated.

Two employees were working together to insert a pin into a hydraulic cylinder. One employee was holding the cylinder in place to align it with the pinhole, while the injured employee was inserting the pin. The injured employee was aligning the pin and moved to determine whether the hole alignment was too high or low. At that moment, the crowbar being used to assist with alignment slipped and his left index fingertip was pinched between the cylinder and the housing, resulting in an amputation just below the fingernail.

An employee was using a circular saw to cut the cap off an aluminum bulkhead of a trailer that was being manufactured. The saw kicked, lacerating his hand and resulting in the amputation of his left middle and ring fingertips.

Two employees were installing skirts on the side of a trailer. The injured employee's left glove was caught by the rotating drill bit that another employee was operating, resulting in a partial amputation to their left little fingertip.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

The Goodyear Tire & Rubber Company

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

Big River Steel, LLC

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
