On or about September 20, 2016, an employee was walking behind a forklift when it backed over the employee's right foot, breaking bones and causing lacerations.

An employee was in the equipment room changing a powered industrial truck battery. While changing the battery, the employee became pinned between the walking jack equipment and battery, resulting in a left ankle fracture.
Two employees were talking in a parking lot. A truck driver, with whom one of the employees had argued earlier, persuaded that employee to go with him to his truck. The driver then picked up a baseball bat and beat the employee, who suffered a broken arm.
