While walking to the trash hopper, an employee stepped over a permanent pipe running along the ground. The employee's foot became caught on the pipe, and the employee stumbled and contacted the trash hopper support frame, breaking the left arm and requiring surgery.

Two employees were drilling a 22-inch diameter hole into the floor of a turbine when the drill bit got stuck. As the employees went to unjam it, the drill became energized and the frame of the drill struck the employees, resulting in a fractured arm, a fractured rib, a laceration to the head, bruises, and abrasions.
