On or about on September 22, 2016, an employee was installing compensation rope to elevator counter weights when a cinderblock brick fell down the 180' elevator hoist way and hit the employee in the head. The employee suffered a broken neck, concussion, and lacerations.

As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members.
