Federal OSHA recorded a severe workplace injury at Otis Elevator Company, 5942 Factory Shops Blvd, ELLENTON, FLORIDA 34222 on — Fractures , affecting the Trunk and other lower extremities.

Employees were installing components of an elevator from inside the elevator and removed temporary hoisting equipment when the elevator descended from the fourth floor to the pit floor. One employee was hospitalized with fractures to their fibula and a rib, and the other employee was hospitalized with fractures to both feet.

An elevator mechanic was lifting a wheelchair lift frame when the load shifted, catching him between the load and an ATM machine. The employee sustained a liver laceration.

An employee had just replaced the passenger-side rear panel of a stake truck and was latching the panels together. A freight truck pulled away from the loading dock and struck the vehicle the employee was working on, pushing the vehicle into the employee and pinning him between the vehicle and a roll-off dumpster. He suffered a hip fracture.

An employee was disconnecting a platform from a shipping crate. The employee's right thumb was crushed between the stabilization bracket on the crate and the platform. The employee was hospitalized for surgery.

An employee was using a magnetic drill to drill a hole on an elevator door. His left glove became entangled with the drill bit, which partially amputated his left thumb below the last knuckle and removed several inches of a tendon. The employee's left middle finger was also severely lacerated. He was hospitalized and received stitches.
