A driver was exiting a tow truck. When he stepped down, he fell to the ground on his hip resulting in a right hip fracture. He was hospitalized.

An employee was walking to and from his semi-truck in a parking lot while looking to pick up a trailer for his next assigned load. The employee was found on the ground next to his truck after slipping on ice and falling. The employee was hospitalized with a fractured skull.

A driver was opening the doors of a trailer. Product fell out and struck the driver, who suffered a broken right ankle and hip.

An employee was closing a butterfly-style trailer door when he fell to the lower step of the trailer and the ground, resulting in torn knee tendons.

A truck driver was manually unhooking a trailer. The driver hooked an extension bar to the manual handle for the fifth wheel and began to pull. The extension bar slipped, causing them to fall backward to the concrete ground and sustain a broken vertebra in the lower back.

An employee was pulling on a bale of corrugated cardboard boxes that were on a conveyor. The straps on the bale failed and the employee fell to the floor. The employee was hospitalized and required surgery for a fractured femur and/or hip.

On December 27, 2023, an employee was delivering mail to an apartment complex when their knee gave out and they fell to the ground. The employee was hospitalized with a left tibia fracture.

An employee was hooking up a set of 28-foot trailers, attaching a converter dolly to the lead trailer's pintle hook. He backed up the lead trailer and parked it a foot from the converter dolly, then lifted the converter dolly and rolled it to the lead trailer to set it onto the hook. The converter dolly kicked up, and the employee's left hand was caught between it and the trailer. He suffered a severe laceration to the palm between the ring finger and middle finger.

An employee was cleaning the windshield of his truck. When he stepped down from the truck, he lost balance and reached to catch himself using the side mirror. The impact fractured his left wrist. The employee was hospitalized and required surgery.

An employee was standing on the DOT bumper on the back of a standard 53-foot trailer. The employee lost his footing and his grip on the handle bar, and fell approximately 42 inches to the ground. The employee was hospitalized with a fractured left hip/femur.

A driver was making a delivery. As he was climbing onto his trailer to get boxes, the strap he was using to pull himself up broke. He fell from the back of the trailer to the ground, landing on his right leg and breaking his femur.

An employee was delivering materials. He was doing his pre-delivery inspection on the roof of the jobsite (a warehouse) when he fell through a plexiglass skylight. He landed on his feet on the concrete floor 14 feet below. The employee sustained fractures to his spine, left ulnar/radius, and right heel. The employee required surgery.

An employee was cutting cabinet trim using a table saw when four fingers on his left hand were severed, resulting in hospitalization and amputation.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
