An employee was operating a pendant crane to move a bundle of 20 sinker bars onto a stack that was eight levels high. As he was unstrapping the hook from the bundle, a banding from another bundle beneath had become undone, causing the bundle of rods to collapse. He was caught in the fallen bundle of rods, causing multiple fractures to both legs. He was hospitalized and had surgery.

An employee was operating a press to remove a cylinder rod from a piston when a spring released its tension, causing the employee's finger to be caught between the piston and the press. The employee's right index finger was amputated up to the first knuckle.

Williams Brothers Construction Co., Inc

A mixer truck driver was delivering a load of concrete to a jobsite. The driver was operating the controls outside of his truck when a 6x10-foot fascia wall gave way. One of the concrete fascia panels struck the driver's right calf resulting in amputation of the leg below the knee.

An employee was installing sheet piles when a pier collapsed and they sustained a broken leg.

M&B Developing Group, Inc.

An employee was receiving a 32-by-8 panel wall that weighed 3,500 pounds. As the crane was going up, the cable of one of the clanks became caught on the panel wall. The panel wall collapsed and struck the employee's right leg, resulting in right leg fractures and amputation below the knee.

Ericsson Smart Factory

An employee was moving conveyors in a warehouse to another location using rolling casters. The legs became detached from the bottom of the conveyor, and the employee's right index finger was caught in the conveyor pinch point, resulting in a fingertip amputation.

Employees were replacing a 24" pig trap door. A strap was shackled on the pig trap door and connected to a forklift. Welders began to cut the door off. An employee was holding onto the door when the cut was finished. The door shifted and caught his fingers, resulting in a hand injury and amputation.

An employee was fitting an arbor fixture into a rotor housing to be reworked. The rotor is moved by a hoist. The arbor was being lowered into the rotor housing when the arbor slipped down onto his hand and amputated a portion of his left thumb.

A crew was tripping out of the hole with an electrical submersible pump (ESP) in the process of swapping tubing tong heads to break down the ESP. An employee's left arm was caught and pinched by the tubing tongs. The employee sustained three lacerations and a fracture to the arm.

On May 21, 2025, at approximately 3:40 PM, a machinist was preparing a CNC lathe machine for it's next run. He was closing the machine door to complete the tooling setup when the door crushed his right middle finger. The employee required a surgical amputation to his fingertip.

An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

An employee was cutting a piece of metal rod with a metal cutting machine. His right middle finger became caught between the rod and the machine's table. The employee's fingertip was partially amputated.

An employee was working at her desk. She went to stand up and fell to the floor beside the desk. The employee sustained a hip displacement and required surgery.
