On July 20, 2019, an employee was not feeling well while performing her normal job duties as a laborer. She went home and was later hospitalized with dehydration.

A mixer truck driver was delivering a load of concrete to a jobsite. The driver was operating the controls outside of his truck when a 6x10-foot fascia wall gave way. One of the concrete fascia panels struck the driver's right calf resulting in amputation of the leg below the knee.

An employee was receiving a 32-by-8 panel wall that weighed 3,500 pounds. As the crane was going up, the cable of one of the clanks became caught on the panel wall. The panel wall collapsed and struck the employee's right leg, resulting in right leg fractures and amputation below the knee.

An employee was moving conveyors in a warehouse to another location using rolling casters. The legs became detached from the bottom of the conveyor, and the employee's right index finger was caught in the conveyor pinch point, resulting in a fingertip amputation.

An employee was participating in demolition work when a non-structural wall collapsed on him. The employee was hospitalized with a fractured shoulder.

An employee was sitting on a stool inspecting products when the stool collapsed. The employee's hand was caught in the collapsing stool, resulting in the partial amputation of a finger.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee was cleaning out an inlet pipe in a stormwater structure. The employee stepped back, fell into the pipe, and landed on concrete 13 feet below, at the bottom of the structure. He suffered a broken back.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was working with paving equipment and heard a noise on the opposite side of the machine he was working on. When the machine stopped, the employee went to the opposite side and crouched down to investigate the noise. A mini track loader backed up and drove over the employee's leg. The employee sustained fractures to their lower leg, ankle, and foot.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

Two employees were setting up a mandrel in the spindle of a machine. The mandrel was side-shifted while an employee's hand was on the mast of a powered industrial truck. The employee suffered crushing injuries that resulted in amputations to the middle and ring fingertips.

An employee had been loading a double deck press. The employee saw a mold had been sent in with the C-hook still attached on the belly bar. As he went to remove the hook from the mold, his right thumb got caught on the C-hook. The employee's thumb was partially amputated.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. An employee was delivering home heating fuel when bees came from the ground and stung the employee's face, neck, torso, and hands. The employee proceeded to shut off the oil flow before losing consciousness. They were hospitalized due to an anaphylactic reaction.
