Final narrative
An employee was working with a coworker to clear some scrap paper that was blocking a sensor on the baler and preventing the baler door from closing. The door could only be raised or lowered using the manual controls. While the employee was clearing the scrap paper from the sensor, his coworker activated the controls to lower the door. The door closed on his left hand, causing multiple metacarpal fractures and lacerations.
Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.
