Federal OSHA recorded a severe workplace injury at The Fishel Company, 2724 Columbus Ave., SPRINGFIELD, OHIO 45503 on — Fractures and other injuries, n.e.c., affecting the multiple face locations.

On October 12, 2016, at 3:10 p.m., two employees were setting up to cut the main 8" gas pipe to install a new pipeline when the pipe coupling failed and the pipe exploded. Debris struck the employees, who suffered facial lacerations and fractures. Both employees were hospitalized.

An employee was standing next to a skid steer, handing tools to the operator. The operator grabbed the tools from the employee and inadvertently hit the controls. The skid steer moved forward and struck the employee. The employee sustained a concussion.

An employee was working on a streetlight fixture to be installed on a pole. The employee was caught between the lower boom of a bucket truck and a tool box, mounted on the bed of the truck. The employee suffered broken ribs and a collapsed lung and was hospitalized.

An aerial crew was lashing a span of fiber telecommunications line when the fiber rose under tension and contacted the overhead primary electrical line. An employee working out of the bucket sustained burns to both hands and was hospitalized.

On 11/7/2018, at approximately 8:45 AM CT, an employee was installing a polymer bell and a shackle between an eye bolt inserted through a pole and energized conductor. He made contact with the hoist that was attached to the energized conductor while attempting to push a cotter pin through the shackle pin. The contact with the hoist caused a phase-to-ground incident, electric shocking the employee. Gloves were not worn at the time.

An employee was using an air compressor on a multi-wheel tire when the flange and the tire burst, causing the wheel's lock ring to strike the employee in the face. He sustained a laceration and facial fractures.

An employee was changing the connection to a compressed nitrogen tank when the system failed and exploded. One employee suffered a concussion and was hospitalized. The other employee sustained soft tissue injuries that did not require hospitalization.

An employee was putting air in a tire that was mounted on a customer's vehicle when the tire exploded, resulting in a left arm fracture that required hospitalization.

An employee was working in a kitchen when a spray can of oil exploded. Hot oil splashed onto the employee, burning their face and arms.

On October 24, 2023, at 12:00 PM, an employee was visually checking a gauge on the "U" controller when a pressure tank exploded. The employee was hospitalized with multiple facial fractures and required surgery.

Bowman Energy Services LLC

An employee was acting as a spotter for a forklift operator. While its forks were being raised, the forklift came into contact with a power line. The employee was touching the forklift's metal frame at the time and was shocked on the left palm. The employee suffered burns to both the left palm and the sole of the left foot.

Performance Contractors, Inc.

An employee was leaning down to install conduit. When he stood up, a 2-inch all-thread rod caught and lacerated his left eyelid.

An employee was working as a fire watch. The employee was exposed to anhydrous ammonia and was not able to see while climbing down a ladder due to eye irritation. The employee fell and was hospitalized for a broken ankle.

An employee was standing on the ground awaiting the lowering of a pipe. After the pipe was lowered to the ground by a skid steer, it rolled and struck the employee's left leg. The employee was hospitalized with a fractured leg.

An employee was tightening a gas pipe with a wrench when the wrench slipped from his hand. The employee's arm then hit a metal cross-brace, resulting in a lacerated arm.

United States Postal Service

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

GXO Logistics

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
