An employee was removing a 24" OD actuator valve that weighed 10,000 pounds from a structure on the fifth floor using a lattice crane. During the lift, the valve became hung up on the permanent handrail and their left thumb was caught between the valve and the handrail. The employee sustained a partial amputation of the thumb.

A crew was using a chain fall system to rig up a hard stop (metal structural support weighing 695 lbs.). The injured employee was guiding it into position. The hard stop shifted and their right hand was caught between the suspended hard stop and the adjacent structural wall, resulting in fractured ring and little fingers.

An employee was walking back from the tool room through the scaffolding material storage area. The employee's pant leg got caught on one of the 8-foot long scaffold planks that was in a scaffold rack. The employee tripped on the scaffold plank and fell forward, landing on his left elbow. The employee sustained a compound fracture to his left arm above the elbow and was hospitalized.

A crew was using rope to move a 24" diameter blind flange from the receiving deck of elevation 4 to a lower location so it could be more easily accessed by a crane. The rope broke and the blind flange fell 15 inches to the receiving deck of elevation 3, landing on the injured employee's feet. Both feet were fractured.

On January 27, 2025, an employee was performing housekeeping. He was moving his toolbox and felt a pain in his lower abdomen. The employee was hospitalized with a hernia.

An employee was putting items away in a locker. They bent down to pick up an item. When they lifted their head, it struck the bottom of a locker. The employee sustained a head injury.

An employee was ascending a fixed ladder and struck his head on an overhead object that was approximately 20 feet above ground level. The employee sustained neck injuries and tingling in his fingers. The employee was hospitalized.

An employee was in the dairy cooler and bent down to get something for a customer. While standing up, the back of his head struck a top stock cart. The employee suffered a brain bleed and required surgery.

An employee was replacing a panel inside a nacelle (wind turbine component) that was at ground level. When he stood up from a crouching position, he struck his head on a metal bracket. The employee was hospitalized with a hematoma on the left side of his head.

The injured employee was helping a co-worker secure finishing material to a pallet using steel strapping (5/8" wide, 0.20" thick). The co-worker went to hand the strapping to the injured employee when they bent down and their right eye contacted the steel strapping, resulting in a laceration to the cornea and injury to the retina.

An employee was acting as a spotter for a forklift operator. While its forks were being raised, the forklift came into contact with a power line. The employee was touching the forklift's metal frame at the time and was shocked on the left palm. The employee suffered burns to both the left palm and the sole of the left foot.

An employee was working as a fire watch. The employee was exposed to anhydrous ammonia and was not able to see while climbing down a ladder due to eye irritation. The employee fell and was hospitalized for a broken ankle.

An employee was standing on the ground awaiting the lowering of a pipe. After the pipe was lowered to the ground by a skid steer, it rolled and struck the employee's left leg. The employee was hospitalized with a fractured leg.

An employee was tightening a gas pipe with a wrench when the wrench slipped from his hand. The employee's arm then hit a metal cross-brace, resulting in a lacerated arm.

An employee was assisting with temporary traffic control setup when they were struck on the right foot by the outrigger of an excavator. The employee sustained a degloving injury to the top joint (knuckle) of the fourth toe.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.

An employee was shot in the leg with a pistol during an altercation with two people who had entered the store without permission.

An employee was directing a truck driver to a dump site when the truck's peanut trailer slipped off the kingpin. As the trailer fell, a ladder attached to it lacerated the employee's ear. The employee was hospitalized.

An employee was unloading a forklift from a trailer to the ground. The employee was climbing down the ladder of the trailer when their foot slipped through one of the rungs, causing them to fall backward onto the forklift forks. The employee was hospitalized for fractured ribs.
