An employee was using a PIT to carry a plastic 55 gallon drum. The drum became wedged between the forks. When the employee tried to maneuver the drum, his pinky finger was caught resulting in a partial amputation.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

Liveo Research, Inc.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was working under a pickup truck, changing out the shift linkage cable. The cable was being attached to the gear shift from within the cab. When the cable was pulled within the cab to connect it, the truck went into neutral and rolled over the employee's left shoulder. The employee suffered fractures to the left humerus, left shoulder blade, and left-side ribs, as well as severe contusions to the left shoulder.

On June 13, 2025, an employee was using a jack stand to lift the front axle of a semi truck. When the jack settled, it slipped off the axle and kicked backward into the employee's leg. The employee suffered a tear in an artery in the leg, as well as a contusion and swelling.
