On October 10, 2016, an employee installed an oil plug on a crowd gate lubricator. He was using a ladder to descend the crowd gate when the ladder moved. He lost his balance, started falling backward, and grabbed the machine's chain, which pulled his finger through the sprocket. The top portion of his left-hand index finger was amputated.

The header dislodged from the payloader, slid forward along with its support stand, and struck both the employee and the customer. The employee was hospitalized with fractures to thoracic and lumbar vertebrae, a fractured rib, and a partially deflated lung. The customer also sustained injury.

An employee was replacing a manure belt in an egg producing poultry house. He was using a mechanical roller and a steel cable to pull (remove) the old manure belt. The employee's ring and little fingertips were pinched between the roller and the cable resulting in partial amputations above the top knuckle.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured.
