An employee was raking parts off the top of a slow moving conveyor (4.5"/minute) when a loud noise startled him and his finger entered a gap between the conveyor and the exit chute. The chute then shifted and dropped pinching his finger and removing his nail and fingertip.

The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.
