Final narrative
An employee was removing lids from a yogurt cup filler while standing on a platform that was approximately 19 inches off the floor. After unloading the lids, she walked down the step leading up to the platform from floor level, lost her footing, and fell into a 24" high stand, breaking two right-side ribs and puncturing her right lung.

An employee was walking from one department to another. She slipped on the concrete floor after walking through a foot foamer (used to protect food quality) and fell on her side. The employee sustained a fractured hip.
The injured employee was waiting near a machine while a maintenance technician was re-installing a pneumatic valve. The valve cycled and the injured employee's left index fingertip was caught in the valve. The employee sustained an amputation of the fingertip.
On August 25, 2024, an employee was servicing a pneumatic valve. He hooked the air supply line to the valve and actuated it, causing the valve to roll. He was stabilizing the valve with his left hand when it closed, amputating his left middle fingertip.
