An employee was shimming a rotor assembly in a lathe to correct a run-out condition before turning the part in a CNC turning lathe. The tail stock foot pedal was actuated, closing the tail stock on and amputating her left index finger to the first knuckle.

Employees were removing dies from a press. The injured employee was guiding a suspended die set by hand to ensure proper placement on the skid when the employee's gloved fingertips were pinched between the upper and lower dies of the blanket die set as it was lowered by a hoist onto a skid. The employee's left middle and ring fingertips were amputated.

Nidec Motor Corporation

An employee was loading large cylindrical arbors vertically into a cart with wheels when an arbor fell. This caused a shift in weight and the basket became top-heavy and tilted. The employee's hand was still on the lifting device connected to the hoist and arbor when the basket and arbors shifted. His left ring finger was caught in a pinch point, resulting in a fingertip amputation.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

Big River Steel, LLC

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.
