An employee was stepping down one step from a cash register area and fell onto a tile floor. The employee suffered a left forehead contusion, subdural bleeding, and injury to their neck and left knee. The employee was hospitalized.

On July 29, 2020, an employee was working on a pump on a water ride. As he was descending a ladder down a rock formation to go into a concrete channel to work on the pump, he tripped and fell approximately 16 feet into the channel, which had been drained of water. The employee's injuries included fractures and dislocations.

An employee was dismantling an elevated platform after the completion of a production. The bolts were removed from a section. When the employee stepped on that section, the floor tipped and the employee fell 7.7 feet landing on his feet and rolling onto his back. The employee suffered bilateral fractures to the ankles and heels.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee was in a paint booth to change filters underneath the booth. She took a step when her right foot fell into an opening in the floor, resulting in a right ankle injury that required hospitalization and surgery.

On October 11, 2025, an employee was walking through a parking lot when they tripped on a pavement joint and fell, resulting in a fractured right ankle and a left knee injury.

An employee was unloading merchandise from a pallet when she tripped over the pallet and fell forward. Her head and face struck the floor of the stockroom, resulting in multiple head and face injuries that required hospitalization.

An employee tripped on an incline/ramp at the door to a building, fell, and suffered a broken left arm.

At the end of a shift on the downhill coaster, an employee was waiting for an empty cart to arrive at the bottom so he could ride it back up the hill. The cart arrived with its seat belt safety lock engaged. The employee boarded the top of the cart and began working to disengage the lock by reaching under the cart. His right hand and two fingers got caught between the rear wheel of the cart and the coaster rail. His ring and little fingers were crushed and lacerated, resulting in surgery and partial amputation of the ring finger.

An employee was readjusting a safety hook to remove hardware from gang forms when his feet slipped. He fell to the ground, resulting in a broken left arm and wrist.

An employee was on a step ladder in a store aisle, making room on shelving for merchandise. She fell from the step ladder to the floor, suffering two fractures in her left leg. She was hospitalized, requiring surgery.

An employee was closing the side cargo door to a company van. The tip of his finger got caught between the door and the door jamb, resulting in a partial amputation.
