Final narrative
An employee was using an automatic tape machine when the machine jammed. As she attempted to free the machine with her left hand, the ribbon cutting shear amputated her index finger from the tip to the first knuckle.
On August 20, 2024, at approximately 12:20 p.m., an employee was performing a metal tapping operation at the metal stamping fabrication station. The employee's left hand was placed on the material being tapped and the rotating bit of the tapping tool caught their left glove, entangling the glove and the employee's left middle finger and resulting in a partial trans-phalangeal amputation and fracture.
Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.
