An employee went to his vehicle in the parking lot to retrieve some items. While walking back to the building, he rolled his right ankle and then stepped forward with his left foot to catch himself. He felt a pop in his ankle and fell to the ground. He was hospitalized for a fractured fibula.
An employee was preparing to disassemble equipment for cleaning by using a metal pick when the pick slipped and punctured the employee's left index finger, resulting in hospitalization.
An employee was pulling on a bale of corrugated cardboard boxes that were on a conveyor. The straps on the bale failed and the employee fell to the floor. The employee was hospitalized and required surgery for a fractured femur and/or hip.
