An employee was cleaning a machine when his sweatshirt was caught and wrapped around the roller bar, causing him to lose circulation in his right arm and wrist.

An employee was loading a flatbed truck. He lost his balance while climbing off the trailer and fell, landing on the concrete floor and suffering a broken right leg. He was hospitalized, requiring surgery.

An employee was rolling a piece of sheet metal on a roller when their gloves got caught in the roller, pulling the employee's right hand into the machine. Skin was removed from two fingers, and the middle finger was amputated at the first knuckle.

While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.
