A forklift was setting 8" non-magnetic collars onto a holder that an employee had placed on the ground. As the employee walked away from the holder, the collars rolled toward him and struck his lower left leg. The employee sustained a fractured ankle.

An employee was lowering a string with a valve attached onto a shaker table when their hand was caught between the guardrail and valve. The employee's right thumb was crushed and required surgery.

An employee was rolling a piece of drilling pipe when his left pinky was caught under the pipe resulting in a fingertip amputation.

As an employee was walking away from the rig floor, he tripped and fell, hitting his shoulder on the drill line that was on the rig floor. His right shoulder blade suffered a hairline fracture.

An employee was rigging down a conductor pipe. A pipe swung and struck the employee in the back as it was being hoisted. The employee was hospitalized with a back injury.

A crew was pulling a 4-inch plastic gas pipe off a reel and straightening it for installation. The injured employee stepped up on the trailer to cut the last band holding the pipe in the coil on the reel. As he turned to step off the trailer, the end of the pipe rotated and sprung out of the cage surrounding the coil, striking the employee on the side of the head and knocking him off of the trailer into the roadway. The employee suffered head trauma that required hospitalization.

An employee and a co-worker were performing a pick inside a clear well. They rigged the skid pan that was full of broken concrete. When lifting, the load began to swing toward a wall. The employee tried to stop the skid pan from swinging and was struck by the pan, resulting in fractures to their left hip and wrist.

The employee had just completed refueling a lattice crane that was breaking up material and was winding the fuel hose back into the fuel truck when he was struck by the catwalk/stairs of the crane. The employee was hospitalized with a laceration to his backside, possible internal bleeding, and a broken hip.

An employee was fusing 10-inch black rubber utility pipes together using a pipe fusion machine and could not get the pipes to set correctly. The employee used a nylon strap attached to an excavator to lift one side of a pipe off a steel plate. As his hand was between two pipes, the pipes came back together, partially amputating two of his fingers.

An employee was lifting four bags of a lime blend weighing approximately 10,000 pounds using a crane. The load swung and pinned the employee against a structural I-beam, resulting in fractures to the hip and pelvis.

An employee was stepping down off a 6-inch containment platform to finish loading his truck, when his right ankle rolled. The employee sustained a fractured tibia and fibula at the right ankle and required surgery.

A service crew was pulling a pump and motor from a well. After breaking a joint of pipe loose from another joint, an employee grabbed the chain tongs to prevent the pipe from spinning in the well, so the crew could continue taking it apart. The bolt that attaches the chain to the handle of the chain tongs broke, causing the chain to swing around and strike the employee's lower right leg. The chain severely punctured his leg and fractured his tibia.

An employee was working to remove a stuck fish tool from a drill shoe. Their thumb was struck by a hammer, resulting in amputation.

An employee arrived on location and loaded a blow-out-preventer (BOP) on a 1-ton flatbed truck. While standing on the flatbed securing the the BOP, the employee stepped into a void/hole on the flatbed surface. This caused him to lose balance and fall off the truck bed with his leg still caught in the hole. As a result, he sustained fractures to his left leg.

An employee was rigging up a location. He was using an Allen wrench to free up rotation and start the drawworks engine manually. The engine rotated and his left little finger became caught between the Allen wrench and the engine. The employee sustained an amputation to his little finger and fractures to his wrist.

An employee fell from a dock to ground level (less than 4 feet) and broke their tibia/knee.

An employee was on the ground working to close a trailer door. The door was caught by the wind and blew the employee backward. He landed on his back and sustained four fractured ribs, requiring hospitalization.

An employee was walking on top of boiler to close a valve. The employee fell off the boiler and landed on the floor, sustaining fractured ribs. The employee was hospitalized.

An employee was exiting a loader and coming down the ladder. His hand slipped off the railing and he fell backward onto sandy ground, landing on his side. The employee was hospitalized with a fractured pelvis and a rotator cuff tear.

An employee's left bicep was lacerated by high-pressure water during water blasting operations. The employee was hospitalized.
