Final narrative
An employee arrived on location and loaded a blow-out-preventer (BOP) on a 1-ton flatbed truck. While standing on the flatbed securing the the BOP, the employee stepped into a void/hole on the flatbed surface. This caused him to lose balance and fall off the truck bed with his leg still caught in the hole. As a result, he sustained fractures to his left leg.

An employee was pulling and positioning the first joint of pipe (30-foot joint of 2-7/8 tubing) at the end of the pipe racks so that it could be lifted at a later time. The pipe was stacked three layers high. The pipe from top layer rolled and three joints of pipe fell at the opposite end of the pipe racks. The employee reached to stop the pipe from falling at the end where he was standing and his right little finger was crushed between the pipe and the pipe rack pin. The employee's finger was amputated.
During maintenance of an annular blowout preventer, air pressure was applied to dislodge a stuck internal piston. The piston ejected and struck an employee's left bicep, causing a laceration and a fracture.
