Employees were repairing a headache rack for a pickup truck. The headache rack was taken off the truck and placed on jack stands; however, the headache rack was not level. The employees attempted to level the headache rack by placing another jack stand under the rack. As the jack stand was being placed under the headache rack, the rack fell off the jack stands. An employee was on top of the headache as it was being leveled. The employee fell off the rack and landed on wooden pallets that were next to the rack. The employee suffered a back injury and was admitted to the hospital.

An employee was driving a street sweeper down a well pad access road when he applied the brakes, slid into a ditch, hit a mound, went airborne, and rolled two times. He suffered fractured ribs, a collapsed lung, and a fractured pelvis.

An employee was using a sledge hammer to install a master pin into the track of a track hoe. When the employee struck the pin with the sledge hammer, a small piece of metal splintered off the pin and struck the employee in the right leg below the knee, puncturing the leg and lacerating an artery.

An employee was working to install a drive belt on a water pump. He was holding the belt on both sides of the pulley, working to route it over the top. As he pulled down with his right hand, the pulley rotated and the belt slid over it, pulling his left little finger between the pulley and the belt. He suffered an amputation to the fingertip as well as a fracture to the ring finger.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.
