This material went over a berm and as the employee ran away from the material, he fell to the ground and was burned by the hot coke and water. The employee was hospitalized.

An employee was climbing down an access ladder on a tanker trailer. He fell approximately 6 feet to the ground.

An employee opened an access panel on a pipe rattler unit to adjust the pipe by hand. His right hand made contact with the pipe cleaning brush, which was still energized at the time, rubbing the skin on his hand away to the muscle layer.

At 10:45 AM on February 17, 2015, an employee's left leg was struck-by a pipe while unloading a truck causing a deep thigh bruise. The employee was hospitalized overnight.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

Employees were conducting a snubbing operation. While making up the tubing swivel onto the joint in the mouse hole, employee 2 proceeded to pull power tongs off the pipe. At the same time, the injured employee went to re-adjust the tong heads. The make/brake handle was pulled and the injured employee's right hand got caught in the tongs. The injured employee's index, middle, and ring fingers were amputated and he required surgery.

On 9/26/2025, an employee was using a pair of scissors to cut a zip-tie that was securing a box of files. The scissors slipped and lacerated the employee s left wrist. The employee was hospitalized.

An employee had been taking fuel samples from the top of a barge. The employee was hospitalized with heat exhaustion.
