Final narrative
An employee was installing form boards inside a confined space cell. While working inside the cell, a piece of pipe (bell reducer) approximately 7" long, 8" diameter, and weighing 55 pounds fell approximately 35 feet and landed on the employee. The employee suffered a neck fracture and a left hand metacarpal fracture with a complex laceration over the web space.
